1095-B forms can be obtained by contacting the RSA Benefits Office and requesting one. Forms can be delivered in person, by mail, or electronically (with your permission). Please contact 951-653-8014 to make a request or if you have any questions regarding the forms. The forms will be provided no later than 30 days from the receipt of your request.
